Below is a list of standard Auto Insurance Claims Forms (OCF Forms) that Ontario automobile insurance companies require you to complete in order to consider your claim for benefits.

These forms and others that you might require are also available on the Financial Services Regulatory Authority of Ontario’s website: fsrao.ca.

The forms can be difficult to understand and must be completed accurately. There are also forms for your doctor (or health care practitioner) and your employer to complete.
